1. Who we are

2. Acceptance of these Terms

By using this site, submitting a form, purchasing, or opting in to text messages, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the site or our services.

3. Eligibility

You must be at least 18 years old and a resident of a U.S. state where we are licensed to provide care. Research Use Only products may be purchased only by qualified purchasers for laboratory research and are not for human or veterinary use.

4. Telehealth services and no medical advice

Content on this site is educational only and is not medical advice, a diagnosis, or a prescription. No provider-patient relationship is formed until you complete intake and are evaluated by a licensed clinician. Treatment is prescribed at the clinician's discretion, and submitting a request does not guarantee a prescription. In an emergency, call 911.

5. Accounts, orders, and payment

Prices, availability, and specifications may change without notice. Orders are subject to acceptance and are processed once payment clears and is matched to the reference shown at checkout. 6. SMS messaging program (A2P 10DLC)

Program description. By providing your mobile number and checking a consent box on our intake form at /book, you may receive text messages from Elevate Chicago. There are two separate, independent opt-ins: (1) automated reminders and service-based messages relating to your consultation, appointments, and care coordination; and (2) marketing and promotional messages.

Consent. Both checkboxes are unchecked by default and optional. Consent is not a condition of any purchase or of receiving care. The exact language you agree to is:

Cost. Message and data rates may apply. Elevate Chicago does not charge for text messages; your carrier's rates apply.

STOP. Reply STOP or OUT to any message to opt out at any time. STOPALL, UNSUBSCRIBE, CANCEL, END, and QUIT are also honored. You will receive one confirmation message and no further messages. Reply START to resubscribe.

Carriers. Carriers are not liable for delayed or undelivered messages. Supported carriers include AT&T, Verizon Wireless, T-Mobile, U.S. Cellular, and most other U.S. carriers.

Privacy of opt-in data. Elevate Chicago does not sell, rent, or share mobile phone numbers or SMS opt-in data with third parties or affiliates for marketing or promotional purposes. 7. Email communications

We send transactional email related to your intake, orders, and care. Marketing email includes an unsubscribe link in every message.

8. Acceptable use

You agree not to misuse the site, submit false information, attempt to access accounts or data that are not yours, scrape or reverse engineer the service, or use it for any unlawful purpose.

9. Intellectual property

All site content, branding, and materials are owned by Elevate Chicago and may not be reproduced without written permission.

10. Disclaimers and limitation of liability

The site is provided "as is" without warranties of any kind. To the maximum extent permitted by law, Elevate Chicago is not liable for indirect, incidental, or consequential damages arising from your use of the site or the messaging program. Nothing here limits liability that cannot be limited by law.

11. Governing law

These Terms are governed by the laws of the State of Illinois, without regard to conflict-of-law rules.

12. Changes

We may update these Terms. Material changes will be reflected by the "last updated" date above. Continued use constitutes acceptance.
