Information we collect

We collect information you submit through forms on this site, including name, email address, mobile phone number, shipping address, intake and health questionnaire responses, and order details. We also collect limited technical data such as IP address, browser type, and the pages you visit, along with the page and referral source associated with your submission.

We collect intake information, clinical history, lab results, communications with our team, and payment information. Together with the identifiers attached to them, these are protected health information (PHI) and are handled under HIPAA.

Separately, we collect ordinary website data: IP address, browser type, pages visited, and referral source. Website analytics data is not treated as PHI and is never combined with your clinical record for advertising.

How we use your information

To respond to inquiries, coordinate care with licensed providers, verify eligibility, process and fulfill orders, send transactional and service-related communications, provide support, prevent fraud, and meet legal and regulatory obligations. With your separate consent, we also send marketing and promotional communications.

To provide care, to communicate with you about your care and your orders, and to operate the practice — eligibility, billing, support, fraud prevention, and legal obligations. We do not sell patient data, and we do not use protected health information for advertising.

Mobile phone numbers and SMS opt-in data

Elevate Chicago does not sell, rent, or share mobile phone numbers or SMS opt-in data with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. No mobile information is shared with third parties for their own marketing purposes. Phone numbers may be disclosed only to the messaging service providers that deliver messages on our behalf, and those providers are contractually restricted to that purpose.

SMS consent is collected through two separate, unchecked-by-default checkboxes on our intake form at /book — one for automated reminders and service-based messages, one for marketing and promotional messages. Consent is optional and is never a condition of purchase or of receiving care. Message frequency varies, up to 6 messages per month; message and data rates may apply. There are two separate consent scopes: care and order messages, and marketing messages. They are independent, neither is a condition of treatment or purchase, and either can be revoked at any time by replying STOP. See our SMS Terms.

Text messaging is not encrypted, and anyone with access to your phone may read the message. We keep clinical detail out of text messages for that reason.

We do not sell, rent, or share mobile phone numbers or SMS opt-in data with third parties for marketing purposes.

How we share information

We do not sell your personal information. We share it only with: licensed providers and pharmacies involved in your care; payment processors; shipping and fulfillment partners; and technology vendors (hosting, email delivery, messaging, and customer records) that process data on our instructions. We may also disclose information when required by law or to protect our legal rights.

Clinicians involved in your care; the laboratory performing your testing; the dispensing pharmacy; and service vendors — hosting, records, messaging, and payment processing — that operate under Business Associate Agreements or equivalent contractual protections. We may also disclose information when required by law.

Health information

Information you share with our clinical team in the course of care is treated as confidential health information and is handled in accordance with applicable law, including HIPAA where it applies. We do not include medication names or clinical details in text messages or email subject lines.

Cookies and analytics

We use cookies and similar technologies to operate the site, remember your cart, and understand aggregate usage. You can control cookies through your browser settings. We do not use mobile opt-in data for advertising.

Data retention and security

We retain information for as long as needed to provide services and to meet legal, tax, and medical recordkeeping requirements, then delete or de-identify it. We use encryption in transit, access controls, and vendor review to protect your information. No system is perfectly secure.

Information is encrypted in transit and at rest, access is limited to workforce members who need it for your care, and system access is audit-logged.

Your choices and rights

You may access, correct, or request deletion of your information, opt out of marketing email via the unsubscribe link, and opt out of text messages by replying STOP or OUT. Depending on your state, you may have additional rights to know, delete, correct, or limit the use of your personal information; we do not sell or share personal information for cross-context behavioral advertising. You may request access to your record, request an amendment, request an accounting of disclosures, and request restrictions on how your information is used or shared. Email frank@elevate-chicago.com with your request; we respond within the timeframes required by law.

Children

Our services are for adults 18 and older. We do not knowingly collect information from children.

Changes to this policy

We may update this policy; material changes will be reflected by the "last updated" date above.
