Nature of telehealth
Elevate Chicago is a 100% telehealth platform. Visits take place by secure video or phone. Some conditions cannot be evaluated adequately without an in-person exam.
No guarantee of prescription
Evaluation by a licensed provider does not guarantee that any medication will be prescribed. Providers exercise independent clinical judgment.
Risks and benefits
Benefits include convenient access to care. Risks include technology failures, delays, and limitations of remote examination. Emergencies should be directed to 911.
Consent
By scheduling a visit, you consent to receive care via telehealth and to the electronic transmission of your health information to your provider and pharmacy.
