Who we are

Elevate Chicago is a physician-led telehealth peptide and hormone optimization practice operating as a HIPAA-covered entity. This policy covers elevate-chicago.com and the services offered through it.

What we collect

We collect intake information, clinical history, lab results, communications with our team, and payment information. Together with the identifiers attached to them, these are protected health information (PHI) and are handled under HIPAA.

Separately, we collect ordinary website data: IP address, browser type, pages visited, and referral source. Website analytics data is not treated as PHI and is never combined with your clinical record for advertising.

How we use it

To provide care, to communicate with you about your care and your orders, and to operate the practice — eligibility, billing, support, fraud prevention, and legal obligations. We do not sell patient data, and we do not use protected health information for advertising.

Who we share it with

Clinicians involved in your care; the laboratory performing your testing; the dispensing pharmacy; and service vendors — hosting, records, messaging, and payment processing — that operate under Business Associate Agreements or equivalent contractual protections. We may also disclose information when required by law.

Text messages

There are two separate consent scopes: care and order messages, and marketing messages. They are independent, neither is a condition of treatment or purchase, and either can be revoked at any time by replying STOP. See our SMS Terms.

Text messaging is not encrypted, and anyone with access to your phone may read the message. We keep clinical detail out of text messages for that reason.

We do not sell, rent, or share mobile phone numbers or SMS opt-in data with third parties for marketing purposes.

Your rights

You may request access to your record, request an amendment, request an accounting of disclosures, and request restrictions on how your information is used or shared. Email frank@elevate-chicago.com with your request; we respond within the timeframes required by law.

Security

Information is encrypted in transit and at rest, access is limited to workforce members who need it for your care, and system access is audit-logged.
